    Description

    Friendship Health Center is hiring a Resident Care Coordinator
    SCHEDULE

    • Monday - Friday
    • Flexible Start Time
    • 8 Hour Shifts
    • Rotating weekends on-call
    HOURLY WAGE
    • $ $61.00 per hour
    • Depends on type of licensure (RN or LPN) and experience
    ESSENTIAL FUNCTIONS
    • Participate in the orientation of new residents/family members to the facility.
    • Admit and discharge residents with documentation regarding care and service provided.
    • Communicate with health care providers in a timely and appropriate manner.
    • Complete daily clinical and environmental rounds to evaluate resident care.
    • Meet and resolve issues with residents and family members. Report significant problems to the supervisor.
    • Maintain a current reference of written nursing material to assist the nursing service department meet the residents' day-to-day needs.
    • Update Care plans.
    • Complete MDS'.
    • Communicate nursing services schedules with other services to ensure the resident's total care regimen continuity.
    • Recommend the equipment needs of the nursing department to the Director of Nursing.
    • Ensure that all nursing personnel comply with the written policies and procedures.
    • Follow clinical programs, including risk management, infection prevention, quality, and staff development.
    • Works collaboratively with other members of the team.
    • Abide by Holgate Center's Code of Ethics.

    QUALIFICATIONS
    • Valid LPN or RN license in the state of Oregon
    • Experience providing direct care in a long-term care, restorative, or geriatric setting as an LPN/RN.
    • Excellent communication and interpersonal skills.
    • Demonstrate good judgment and decision-making skills.
