Job Description:


Collaborates with a Medical Doctor to assess and screen admissions, ensuring the appropriate level of care. Provides recommendations for different care options when a patient may not require inpatient hospitalization.

Interacts with patients, families, and other healthcare professionals to create safe discharge plans. Acts as a patient advocate while managing healthcare services efficiently and cost-effectively without compromising quality. Arranges transfers to healthcare facilities as needed and complies with healthcare regulations.



Key Responsibilities:

  • Plans, evaluates, and oversees care for patients.
  • Works with the healthcare team to develop individualized care plans.
  • Recommends alternative care options to meet patient needs.
  • Assesses high-risk patients requiring post-hospital care.
  • Coordinates discharge planning and communicates effectively with all involved parties.
  • Reviews and monitors patient hospital stays to ensure timely services.
  • Facilitates continuity of care through various services.
  • Conducts clinical reviews based on guidelines for different patient settings.
  • Acts as a liaison between facilities and provides case management.
  • Refers patients to community resources as needed.
  • Educates healthcare team members on discharge planning and resource usage.
  • Provides patient education and support for post-discharge needs.
  • Reports any unusual occurrences or risks related to patient care.
  • Analyzes utilization patterns and participates in data collection and studies.
  • Participates in care planning rounds and committees.

Requirements:

  • Two (2) years of RN experience in acute care or case management.
  • Completion of an accredited RN training program.
  • Registered Nurse License (California) and Basic Life Support certification.
  • Familiarity with regulatory and accreditation standards.
  • Strong communication, critical thinking, and organizational skills.
  • Computer literacy.

Preferred Qualifications:

  • Bachelor's degree in nursing or a related field.

Notes:

  • Block schedule, with shifts every other weekend.

California, Los Angeles Medical Center


Work Hours:

Part-time, Day shift; Sun-Sat, 24 hours per week.
