Job DescriptionNortheast Counseling Services is searching for a caring, compassionate person who desires a rewarding and meaningful employment life helping people with mental illness take charge of their lives and become productive citizens. The work is done in communities settings, not in offices.
The Blended Case Manager works with the serious and persistently mentally ill population, using a strength based perspective, to assist clients to obtain necessary services and support. The primary goal is to empower the client to identify their needs and choose the resources to fulfill them. This workforce member has access to the complete chart for cases requiring his/her workers involvement.
Educational Requirements:
High School diploma and 12 credits in Social Service field with two year's experience.
OR
Bachelor's degree in social work, psychology, or related field is necessary.
Must maintain a valid PA Driver's license.
