***Responsible for coordinating case management needs of residential participants. Serves as a liaison with Director/Facilities on facility issues. Assists supervisor in completing clinical documentation. In consultation with Director, serves as point person for the residential program in the event of supervisor's absence. Responsible for providing rehabilitative residential support services for adults with severe mental illness or dually diagnosed. Teaches in-vivo independent, social, and daily living skills taking into consideration the participant's learning style, strengths and needs. Monitors participants' symptoms and medication utilization regularly. Maintains an organized, clean, and safe home environment. Provides therapeutic interventions that facilitate illness self-management, skill building, identification of natural supports, and use of community resources. Provides crisis management services in consultation with program supervisor. Documents all services using program protocols.


REQUIREMENTS

  • Bachelor's degree in psychology, social work, or related field.
  • Experience working with persons with psychiatric or other disabilities.
  • Experience working with diverse ethnic populations required.
  • English/Spanish bilingual preferred.
  • Flexibility to work on weekends, evenings, holidays during emergencies.

Schedule:

  • 8 hour shift
  • Monday to Friday
  • Weekends as needed
