- Evaluating patients' functional statuses, such as muscle function, endurance, visual coordination, communication skills, and self-care abilities
- Assessing the home environment for potential hazards or barriers to independent living and determining equipment requirements
- Contributing to the development of personalized care plans
- Creating treatment programs and setting goals for enhanced functionality. Collaborating with the physician and other team members on the care plan
- Teaching patients new skills or retraining them in daily activities affected by illness or injury
- Ensuring accurate clinical documentation and promptly reporting any changes in patient status to the physician
- Fabricating splints and educating patients on the use of adaptive equipment to enhance function
- Training patients on the application of prosthetic and/or orthotic devices
- Identifying patients' and their families' needs for additional home health services and making appropriate referrals
