SUMMARY:


The Community Health Worker (CHW) offers preventive health services to prevent disease, disability, and other health conditions or their progression; to prolong life; and promote physical and mental health for patients and the community served.

The CHW assists patients, their families, and community members to navigate and access community services and adopt healthy behaviors and works cohesively with internal departments to achieve integral and coordinated care.

These services include but are not limited to patient education and linkages to community resources with the aim of educating and empowering patients and their families to control symptoms and thrive.

The CHW serves as a navigator to link healthcare and social services to the community through a culturally responsive lens to improve care.


ESSENTIAL JOB DUTIES AND RESPONSIBILITIES:


  • Supports and implements the organization's vision, mission and values.
  • Determines priorities and method of completing daily workload to ensure that all responsibilities are carried out in a timely manner.
  • Performs all job functions in a professional and courteous manner. This includes answering all general phone calls timely and providing excellent customer service to internal and external customers.
  • Fosters and promotes a culture of service excellence and accountability.
  • Conducts outreach activities to identify community members who may benefit from health education and support services. Provides culturally appropriate health education to individuals and families in the community, including topics such as chronic disease management, healthy lifestyle choices, and preventative health measures.
  • Advocates for community members' needs and connects them with community resources and services that address the social determinants of health, such as housing, food, and transportation.
  • Supports participants to enroll or maintain enrollment in Medi-Cal or other assistance programs that are related to improving their health if such navigation services are provided pursuant to a plan of care. Outreaches to potential members from a call list, informs them of programs and benefits, and enrolls them in services.
  • Acts as a liaison or bridge between patients/families and health providers/professionals.
  • Provides patient education to patients/families in both individual and group settings.
  • Attends assigned community events and meetings to represent the organization and support health center visibility and enrollment.
  • Acts as an advocate in the community on behalf of patients/families to assist in resolving barriers that may interfere with health promotion, disease prevention, and general wellbeing.
  • Assists in maintaining and improving processes and systems that ensure comprehensive and thorough patient care (i.e. documents every patient visit, followup appointments scheduled, etc.).
  • Complies with organizational policies and procedures.
  • Performs all other duties as assigned including participation in special projects and research.
  • Must be willing and able to work at all locations as needed to meet patient care needs.
  • Must be willing and able to work all business hours including evenings and weekends.

QUALIFICATIONS:

EDUCATION/EXPERIENCE:

  • Lived experience in communities for this program and/or one year of communitybased direct service experience.
  • Community Health Worker Certification Required and/or ability to complete CHW Certificate Program within 6 months of employment.
  • Bilingual English and Spanish required.
  • Valid California driver license, good driving record and proof of car insurance.
  • Available to work evenings and weekends.

LANGUAGE SKILLS:
Ability to read and interpret documents, such as policies and procedures, routine mail, simple contracts, and procedure manuals. Ability to compose routine reports and correspondence. Ability to speak effectively with employees, visitors and management.
