CARES Case Management:

  • Maintains a caseload of an average of 40 Residents.
  • Conducts assessments with residents to identify the psychosocial, economic, and physical needs of the household.
  • Provides information and referrals, as appropriate, and supports residents in connecting to appropriate resources.
  • Develops and implements individual service plans and assesses and reassesses residents' support systems and available community resources.
  • Provides case management and maintains documentation to accurately evaluate the resident's progress and ongoing resident and household needs.
  • Supports residents in the stabilization of biopsychosocial functioning and economic needs.
  • Facilitates resident contact and interdisciplinary communications on resident behalf.
  • Provides shortterm crisis counseling and support.
  • Provides closure planning to residents exiting the CARES case management program.
  • Participate in special projects as assigned by supervisor.
  • Facilitate ERAP Consultations with CARES residents needing assistance with Emergency Rental Assistance Programs.
  • Maintains knowledge of best practices in human/social services.
  • Participates in organizational workgroups.

What you'll need to have:

  • Bachelor's degree from an accredited college or university with a major specialization in Social Work or a Human/Behavioral/Social Services related field OR Master's degree from an accredited college or university with major course work in a field related to Social Work
  • Minimum of two (2) years of experience in a field related to counseling individuals and/or case management services.
  • Knowledge of crisis intervention and counseling techniques used in trauma, grief, or crisis situations.
  • Skill in effective oral and/or written communication.
  • Skill in establishing and maintaining good working relationships with other social service programs.
  • Skill in preparing clear and concise written reports.
  • Ability to learn and understand regulations across the MSR platform which includes 17 states.
  • Ability to develop diplomatic relationships across multiple communities, within and external to the company.
  • Ability to interact with residents, interns, and organization peers in a strengthbased, solutionfocused manner.
  • Ability to acquire and maintain knowledge of community resources available for basic needs, therapeutic interventions, and/or psychosocial support.

Preferred:

  • Bilingual proficiency in English and Spanish
  • Skill in motivational interviewing and solutionfocused techniques
  • Knowledge of Federal and State program requirements for services to adults and families.
  • Knowledge of theoretical constructs of human development and ecological systems.
  • Knowledge of evidencebased interventions and counseling techniques used in trauma, grief, or crises.
