Description:

This role involves providing skilled nursing services to patients battling substance use disorders.

The nurse is responsible for continuous assessment, evaluations, triage, harm reduction teaching, stabilization, and monitoring of patients within an ambulatory community-based treatment model.

The nurse collaborates with primary care providers, an integrated treatment team, and community providers to develop goal-oriented treatment plans and provide support and services to patients referred to the program for stabilization and assessment.


Responsibilities:

Ongoing Assessment and Evaluation: Conduct comprehensive and ongoing assessments of patients to understand their unique needs, identify any co-occurring conditions, assess their readiness for change, and monitor their progress over time.

Triage and Stabilization: Triage patients based on their level of need and provide immediate interventions to stabilize acute issues related to substance use, such as overdose or withdrawal symptoms.

Harm Reduction Teaching: Educate patients on harm reduction strategies to minimize the negative consequences of substance use.

Treatment Plan Development: Collaborate with patients to develop individualized, goal-oriented treatment plans that address their unique needs and recovery goals.

Collaboration with Care Providers and Treatment Team: Work closely with primary care providers and the integrated treatment team to ensure holistic care for patients. This may involve coordinating medical care, mental health services, social support, and other resources.

Referral Coordination: Facilitate referrals to other healthcare providers, community resources, and support services as needed to address the broader needs of patients and support their recovery journey.

Collaboration with Outside Referral Sites: Liaise with external referral sites, such as addiction treatment centers and community health settings, to ensure seamless transitions of care and support continuity for patients.

Education, Experience, Licensing

  • Requires any combination of education and experience equivalent to an Associate's Degree or specialized courses/training equivalent to satisfactory completion of two years of college in nursing science or closely related field and two years nursing or closely related experience.


  • Registered Nurse (RN)

  • State of Florida or Compact State Nursing License.
  • Possession of a valid driver's license and acceptable driving record.
  • Current BLS for Healthcare Professionals.
  • Current Advanced Life Support Provider.
  • Certified Addictions Registered Nurse (CARN) or the ability to be certified within eighteen months of hire.
Supplemental Information

  • Work environment includes both indoor and outdoor settings.
  • Office work may require sitting for extended periods, working on computers, and making numerous phone calls.
  • The ability to move and lift equipment up to 50 lbs is required.
  • Fieldwork conditions may be noisy, uncomfortable, unkempt, and stressful with occasional exposure to blood or other potentially infectious materials (OPIM).
  • Regularly required to drive county vehicles and to work nonstandard business hours, including evenings, weekends, and holidays.

Pay:
$42, $85,812.22 per year
