Job Description:
The Physical Therapist (PT) is responsible for evaluating and treating residents under the orders of a physician.
This includes using various therapeutic procedures to restore muscle strength, coordination, body alignment, relieve pain, and improve movement and function.
As a member of the interdisciplinary team, the Physical Therapist provides support and information within the field of Physical Therapy.
Responsibilities:
Review resident information and assess its impact on treatment process.
Conduct a thorough functional assessment of residents, including home/vocational and equipment needs.
Create and implement individualized treatment plans with achievable functional goals.
Adhere to evaluation, treatment, and documentation guidelines, maintaining accurate and timely records.
Instruct and supervise Assistants in treatment procedures.
Document treatment progress in resident records.
Revise treatment plans as necessary.
Prepare clear and concise discharge summaries.
Orient residents to program services and inform of any potential risks.
Collaborate with nursing staff and families to maximize functional abilities.
Instruct residents and caregivers on post-treatment exercise programs.
Assist in the referral process for additional services.
Participate in team conferences and other department meetings.
Pursue continuing education opportunities to maintain clinical competency.
Contribute to the quality improvement process.
Promote programs and services through interactions with the community.
Adhere to regulatory and professional standards.
Provide input on clinical performance of assistants to the Rehab Manager.
Follow facility policies and procedures for quality improvement and safety.
Perform additional duties as assigned to provide quality care.
Understand reimbursement guidelines and allocate therapy resources accordingly.
Comply with regulations such as OBRA, HIPAA, and Local Coverage Determination.
Support the company's commitment to Compassionate Care.
Work independently and recognize situations requiring supervision.
Maintain expected productivity levels.
Qualifications:
Bachelor/Masters of Science degree in Physical Therapy.
Current license in the state of practice.
Prefer supervision, training, or experience in a similar setting.
Certification in specialized areas if applicable.
Excellent oral and written communication skills.
Competency in applying physical therapy principles, methods, materials, and equipment.
Ability to assess resident needs and develop comprehensive care plans.
Knowledge of accreditation standards and compliance requirements.
