Certified Home Health Aide (CHHA)
Sonata Hospice

Sonata Hospice provides professional medical care and volunteer support to people with life limiting illness.

Our goal is to provide excellent care so that our patients remain comfortable in their own homes and stay as independent as possible.


General Responsibilities:


  • Provides personal care to patients under direction of a Registered Nurse
  • Care provided in patient homes, RCFEs (assisted livings and board and cares), nursing homes
  • Frequent updates to RN Case manager on patients' condition
  • Other duties as assigned
  • Must have current California CHHA Certificate
  • Valid California Driver's license
  • Reliable transportation
  • Good communication skills
  • Hospice and Pointcare preferred
Pay Range

$25-$30/hour
