Intake Specialist - TEEMA Group, Inglewood, CA, USA
The Intake Specialist will be responsible for providing excellent customer service and support to our clients. This role requires a highly organized individual with excellent communication and problem-solving skills.
Responsibilities:
- Performs client eligibility screening and assessment.
- Aids clients in gathering required information for program enrollment.
- Handles client intake procedures, completes intake packets, and enters intake and discharge data into the program database, along with other necessary client details for reporting.
- Conducts individual or group program orientations for clients.
- Electronically tracks client entries and exits, inputting data into the program database and, if applicable, into the Homeless Management Information System (HMIS).
- Ensures MyCaseRecords (MCR) documentation is kept current.
- Contributes to the creation of client information and program data reports, including monthly, quarterly, and annual reports, utilizing MCR and HMIS data.
- Collaborates with Program Coordinator, Case Managers, and Housing/Outreach Specialists in monitoring and documenting services.
- Assists in maintaining archives of inactive client files.
- Maintains ongoing commitment to quality documentation practices.
- Offers direct and confidential support to management and staff.
- Assists in procuring and managing necessary program supplies and office equipment.
- Communicates program guidelines to clients and community partners.
- Supports monthly aftercare followup procedures.
- Perform other duties as assigned.
Qualifications:
- Bachelors degree in social services field or a related field preferred.
- One year of experience in social services or related field required.
- Valid drivers license is required
