Client Care Coordinator
In addition, managing clients' Care Plan and supervising direct care workers' quality care every quarter.
Client Care Coordinator will perform a Hybrid-remote schedule to support in-home care services and ensure clients are receiving the best care possible in the most comfortable setting in their home.
Responsibilities:
Successfully build relationships with clients, case managers, and caregivers.
Supervisory field visits to clients' homes.
Provide administrative support to management team and owners
Work in coordination with Recruitment and Retention team
Serve as a first point of contact for clients, case managers, and caregivers
Professionally and efficiently understand care and scheduling needs
Maintain efficient knowledge of requirements and preferences of each case
Coordinate and confirm accepted assignments with both clients, case managers and caregivers
Coordinate training required for caregivers prior to their date of hire
Ensure all placements have a perfect match experience and practice standard safety practices
Document all client, case manager, and caregiver correspondences
Ensures client satisfaction by performing supervisory field visits
Provide debriefing on staffing shortages and needs
Ensure accuracy of caregiver files and information
Teamwork:
Work cooperatively in a team environment providing feedback and updates as required.
Adhere to all company regulations, quality, and safety standards.
Perform additional responsibilities as needed.
Skills/Abilities:
Ability to support the lives of others in a compassionate and supportive manner.
Must be able to adapt to changing situations as needed.
Must be able to follow instructions and implement proper safety protocols and standards.
Ability to act in emergency situations quickly and responsively.
Knowledge of AHCCCS/Medicaid
Experience in home health or home care industry
Administrative experience
Customer Service experience
Great phone skills
Requirements:
High School diploma or GED equivalency.
Computer Proficiency
Driver's License
Legally eligible to become a W-2 employee
Vehicle to drive to clients' homes
Competencies:
