Prevention Family Support Worker - Sarasota, United States - Safe Children Coalition
Description
PURPOSE:
Works with families that have had an abuse report/investigation with allegations of abuse, neglect; or with children of families identified as needing services.
The focus will be partnering with investigations and ERAT counselors; working in the home and engaging the family with linkage to community services and interventions that will manage danger threats, increase safety and support the child(ren) remaining in the home while providing support to the family in accessing services and interventions.
KEY RESPONSIBILITIES:
Provide families with referrals and linkages to community services.
Assist the family with regard to timely follow through on referrals provided.
Coordinate services with other professionals, paraprofessionals, and other community/familial supports.
Participate in home-based family team meetings with family and community partners that are involved with the family.
Be familiar with and knowledgeable of services and providers within community that provide and offer servies and supports to meet families' needs.
Arrange or provide transportation of clients when needed. Provide on call support as required or scheduled. Participate in Quality Improvement process. Maintain professional and respectful interaction with family and any identified support to the family. Function as a contributing member of the multi-agency team as needed. Participate in team staff meetings. Develop and maintain case documentations according to agency standards. Attends staff development training for professional growth.Document in data systems on a timely basis as required and needed.
Provide back-up support to the ERAT staff as directed.
Assist with safety plan monitoring as needed.
RequirementsLIST OTHER MINIMUM REQUIREMENTS:
Possess a valid Florida Driver's License in good standing and be insurable by the agency's current insurance carrier. Must meet state criminal background check requirements. Ability to work a flexible schedule. Knowledge of community resources.
EDUCATION / EXPERIENCE:
Minimum of High School Diploma or GED equivalent; two years direct experience in the delivery of services to families and children.
