Housing Support Care Manager
_Supervisor Title_: Case Manager Supervisor

Under the Permanent Supportive Housing Program, the Case Manager will also be responsible for assisting the individuals and families with finding and retaining housing, maintaining landlord relationships, leasing up and identifying sources for housing funding, education, counseling and linkage to appropriate related community resources.

Educational Requirement_**:
Bachelors Mandatory

Licensure and/or Certification_** First Aid and CPR; Valid Driver's License and Auto Insurance.

Required Experience_**:

At least three years' supervisory experience, to include demonstrated leadership, familiarity with operational, financial, quality assurance, and compliance related operations of the organization.

Knowledge of current evidence based practices and field implementation of those practices. Knowledge of HUD (Home First Model), DHS (PSHP), DCHA (HCVP) and DBH (MHRS/Community Support) regulations a plus. Teamwork oriented, flexibility in working assignments and locations.

Duties:


  • Provide strong case management services to chronically homeless individuals and families.
  • Implement Permanent Supportive Housing Program for chronically homeless in alignment with Housing First model.
  • Ensure compliance with case management standards as delineated in HUD's Housing First Initiative.
Case management and recordkeeping includes, but is not limited to:

intake; service planning; service delivery including advocating for clients with property owners/managers as necessary and providing resource information; and case closure.


  • Develop Housing Service Plans, that are person-centered with program-specific goals, objective, interventions and outcomes to ensure program effectiveness and impact. Manage all program services directly related to those outcomes.
  • Develop and manage a comprehensive outreach/ working relationship with stakeholders including other community members, especially landlords. Implement a landlord recruitment and retention plan. Provide recommendations to deal with expected changes.
  • Coordinate case management services with other human service providers assisting clients.
  • Develop new program components as directed and assist in provide on-going training classes on related housing topics.
  • Maintain materials, equipment, and forms necessary for program operations.
  • Perform home visits and off-site case management meetings.
  • Participate in internal and external training classes, which may be required by accreditation standards.
  • Ability to multi-task, but maintain high performance and quality standards.
  • Other duties as deemed necessary.
