Care Coordinator III (Glendale Adult Services)
This position is partially remote, and you have the option to elect into a 9/80 flex work schedule. The salary for this position is between $66,600 - $69,134 annually.

_ Summary_


We are seeking a Care Coordinator to provide case management, crisis intervention and linkage to community resources for individuals with a history of persistent mental illness and/or substance use; ensures clients receive the maximum benefit from all interventions in an integrated health care setting.

This position will also provide supervision to three to four care coordinators and assist with other administrative projects, as appropriate.

This position will perform job duties while ensuring a safe, nurturing environment for clients and staff that fosters a sense of hope, dignity and recovery.


_ Primary Duties_

  • Oversees care coordinators in the daytoday operations of the program and/or Division

Including:
managing schedules and reviews and approves timesheets in timekeeping system.

  • Provides supervision to care coordinators including supporting training needs and skill development, upholding expectations and providing direction as needed.
  • Collaborates with supervisors across the Division to ensure Agency needs are meet related to the care coordinators.
  • Conducts care coordinators team meetings and trainings as appropriate.
  • Assists with additional Divisionwide projects as assigned.
  • Assists clients to locate, access and utilize a full range of health care services, community support groups and resources; establish and maintain housing; access mental and medical care, rehabilitation programs and drug/alcohol services; increase social contacts and engage in constructive activities.
  • Assesses clients' needs and functioning, presents cases in treatment team meetings and monitors progress as needed.
  • Acts as client advocate interfacing with housing and support agencies including property owners, support groups, Social Security, Medicare/MediCal, etc.
  • Develops and maintains rapport and cooperative relationships with community agencies for the benefit of the clients

  • Provides individual/group supportive counseling and psychoeducation to assist clients in coping with mental illness and/or substance use, promote recovery and develop selfsufficiency.
  • Provides crisis intervention and facilitates conflict resolution among the program clients as needed; assists clients in problemsolving life situations.

  • Monitors and maintains documentation of client services; ensures compliance with Agency, county, state and federal policies and procedures, standards and guidelines.
  • Develops and maintains clinical skills through seminars, workshops, or other forms of continuing education/training related to individuals with a history of mental illness and/or substance use disorders.
  • Attends and participates in Agency and program/team meetings, individual/group supervision, and related community Agency meetings as required/requested.
  • Participates in community resource and/or health fairs.
Secondary Duties

  • Not use or disclose protected health information about any member or other party in compliance with Didi Hirsch's policies related to state or federal laws such as Health Insurance Portability and Accountability Act (HIPAA).
  • Use appropriate safeguards to protect the confidentiality of such information.
  • Report to management any use or disclosure of protected health information not permitted by Agency policies related to state or federal laws such as HIPAA.
  • Participate in training or briefings on HIPAA information as scheduled.
  • Consult with supervisor on any issues or questions about compliance under Didi Hirsch's policies related to state or federal laws such as HIPAA.
Supervisory Responsibility

  • The employee in this position will supervise three to four care coordinators.
