- Assists with referral intake process to share program information with patients and families.
- Collects information and creates the referral according to policies and procedures.
- Develops and maintains a hospice presence in hospitals, long-term care facilities, and other targeted referral sources by coordinating with providers, case managers, and other staff that have direct involvement in the referrals and admissions process.
- Manages initial assessments in the home, facility, hospital, or any other settings as needed to evaluate physical, environmental/safety needs of patient to explain services and to develop an initial plan of care in collaboration with patient, significant others, and interdisciplinary team members in order to assure satisfactory patient outcome.
- Evaluates patient's eligibility for program services. Communicates with primary physician regarding assessment findings and initiation of treatment plan.
- Presents new patients to members of the interdisciplinary team.
- AS or BS Degree in Nursing from an accredited school of nursing
- Current NC state license as a Registered Nurse
- Ability to complete time sensitive documentation on a daily basis.
- 2-4 years of experience as an RN required.
- CHPN is preferred.
- Valid driver's license and reliable independent transportation.

Gpac is partnered with a thriving hospice organization looking for a Registered Nurse Liaison with experience or an interest in palliative and hospice care. This is an onsite role in the High Point area. The hours will be Monday - Friday 8:00AM – 5:00PM.
Overview
This Nurse Liaison (RN) position is primarily responsible for assisting the Referrals and Admissions Department in admitting patients onto agency services including hospice, palliative care and other programs. The Nurse Liaison participates in admissions, preassessments, and other related visits across various settings including the home, inpatient units, hospitals, long-term care facilities, or all other settings as necessary. The Nurse Liaison shall assure appropriateness for services, verify orders, and communicate with other members of the interdisciplinary team to develop a plan of care.

Requirements
Salary: $70,000 - $80,000/year
Benefits: Comprehensive Benefits Package. BCBSNC medical, Ameritas dental and vision, 403b retirement plan, 22 days of PTO a year for FT employees.
Location: High Point, NC. Travel each day to hospitals, long-term care facilities and homes within a 35 mile service radius. Mileage reimbursement.
Hours: Monday – Friday. 8:00AM-5:00PM
