- Provide case management services in accordance to the Preferred Communities Program Plan, including:
- Complete initial assessment within 7 days of client enrollment
- Complete interim assessment every 90 days
- Complete closing assessment and submit within 14 days of case closure date
- Record external referrals in agency client database system
- Develop self-sufficiency service plans in conjunction with client
- Meet with clients on caseload at least once every 30 days (but more often as appropriate for clients with exceptional needs) for at least 6-12 months or until self-sufficiency is reached
- Maintain a caseload of at least fifteen (15) clients and provide services to meet their needs including, but not limited to, medical, mental health, housing, and other areas of support.
- Maintain clients' paper and digital case file by creating and reviewing case notes, completing assessments, and logging case events and progress on goals.
- Ensure case files, both paper and electronic, are kept up to date with the required documentation as set forth in the Cooperative Agreement and program guidelines.
- Complete case notes for all client interactions and ensure information is entered into the program database in a timely manner per documentation policy procedures.
- Collaborate with staff members from other departments as well as community providers to support the transition to self-sufficiency.
- Work closely with community partners as well as establish new partnerships and linkages in the community to ensure clients are connected with all services needed to achieve self-sufficiency goals.
- Work collaboratively with PC clients to support goal-setting, skill-building, self-advocacy, problem-solving, navigating community resources, and creating support networks that sustain and maintain clients' self-sufficiency goals.
- Attend agency and department meetings.
- Connect clients to community resources to help address needs and benefit their daily living (i.e. food pantries, thrift stores, faith communities)
- Maintain communication and cultural understanding with clients, between clients and partners, the Agency, and the community.
- Work collaboratively with student interns and volunteers.
- Assist with preparation of program reports.
- Collaborate with Refugee Resettlement Services staff as needed to ensure that clients are receiving the services they need to achieve their goals.
- Travel up to 100 miles from MEIRS office to meet clients, use of personal vehicle required
- Other duties as assigned
- Bachelor degree in social work or related field preferred but not required
- At least one year of experience working in a social services or health-related field, providing direct services as a caseworker, social worker, care manager, etc.
- Demonstrated ability to conduct client assessments, develop service plans, implement interventions, and evaluate service effectiveness
- Knowledge of local health care system and community resources
- Experience with refugee/immigrant/new Mainer populations preferred
- Demonstrated ability to successfully resolve challenging problems through use of critical thinking skills in collaboration with clients, colleagues, and partners
- Ability to accurately and clearly document work through written case notes
- Cultural humility and sensitivity to the wide range of cultural, ethnic, linguistic, religious or other backgrounds of clients, colleagues and partners
- Strong oral and written communication skills
- Excellent interpersonal and active listening skills
- Ability to communicate with empathy and respect with clients, colleagues and partners

Job Summary:
The Preferred Communities (PC) Intensive Case Manager (ICM) works closely with Direct Supervisor and other team members of the resettlement staff to achieve the successful resettlement and integration of especially vulnerable refugee groups and other ORR-eligible client populations. The PC-ICM assesses client needs, creates a Self Sufficiency Plan in collaboration with the client to address barriers to the individual's successful integration into their new community; and takes active steps in supporting the achievement of the established goals within one to four years. Case Management services can include referrals and other community linkages; support to community partners in providing services that meet clients' needs; and educating clients on how to navigate the complicated systems they often interact with. A flexible, hybrid work schedule is available once trained and working independently on the job. This position carries a travel component as the PC-ICM serves clients within a 100 mile radius of the MEIRS offices in Lewiston Maine, travel in PC-ICM's personal vehicle is required.
