We are an accredited substance abuse and mental health outpatient program, seeking dynamic, passionate, and Case Manager/Group Facilitator for Day or Evening shift I.O.P.

Job Summary:

The Case Manager is responsible for motivating patients to engage in and participate in all elements of the treatment process, including education groups, treatment planning, individual counseling, and understanding the disease process.

The role involves providing support to patients in overcoming addiction and assisting them in developing effective coping strategies.


Duties and Responsibilities:

Group Counseling:

Conduct individual and group counseling/case management sessions to discuss effective coping skills and assist clients in identifying and overcoming addiction-related problems.

• Client Assessments and

Treatment Planning:

Conduct client psychosocial assessments, develop and monitor individualized treatment plans, carry out client intake processes, and address any pertinent issues.

• Group Facilitation: Conduct diverse group therapy sessions that foster a supportive environment for recovery. Monitor and record client engagement and progress, addressing any treatment goals or interventions observed during sessions.
• Collaboration: Collaborate with a treatment team comprised of clinical staff, including the clinical director and treatment milieu, to ensure cohesive patient care.
• Behavioral Monitoring: Identify patients' negative behaviors by asking personal questions and monitoring their actions to provide appropriate interventions.
• Addiction Support: Assist patients in living more effectively with their addictions by identifying harmful behaviors and helping them overcome urges.
• Patient Documentation: Demonstrate an understanding of patient care through charting and documenting patients' progress in Kipu EMR and provide service referrals when needed.
• Communication: Maintain open and effective communication with case managers and the program director regarding client updates and group dynamics. Ensure that any challenges or breakthroughs in the group setting are promptly communicated to facilitate client care.
• Assessment Review: Review assessment material, report relevant symptomology, and assign clients to therapists, psychiatrists, referents, and other adjunct providers to gather comprehensive information regarding client presentation and appropriateness for care.
• Information Exchange: Ensure ROI (Release of Information) is obtained by staff to facilitate the exchange of information.
• Incident Reporting: Ensure incident reports are completed within the required deadlines.
• Patient Engagement: Encourage clients to engage in their treatment and adhere to group and program rules.
• Utilization Review Assistance: Assist with Utilization Review when necessary by providing additional information regarding client symptom presentation.
• Other Duties: Perform any other duties as needed.

Qualifications:

Possession of a recognized active certification in drug and alcohol counseling (CADC, CATC, LPCC, LAADAC) or registration through BBS (MFT, AMFT, ACSW).

• At least two (2) years of professional experience in the addiction field.
• Current certifications in CPR and First Aid.
• Current Health Screen and TB Test.
• Experience in addiction counseling and case management.
• Proficiency in using EMR systems, particularly Kipu EMR.
• Strong interpersonal and communication skills.
• Ability to work collaboratively within a multidisciplinary team.
• Excellent organizational and time management skills.

Work Environment:

• The role is primarily performed in a clinical setting.
• Choose a preferred schedule: Day or Evening Schedule.

Reporting Structure:

• Reports to the Clinical Director.
