***The Resource Coordinator is responsible for the delivery of quality patient care. Works to remove barriers to care by direct contact and relationship building with identified clients and referral agencies. Follows up with clients in person and via telephone to ensure adequate use of services and connections with community resources. Provides screening, assessment, evaluation of needs, care management plan development, and linkage to higher levels of care for patients interested in recovery or emotional support. Works with a multidisciplinary care management team Collects relevant data and generates reports on case management activities for use internally and to meet grant maker requirements. This position reports to the Chief Nursing Officer.

PRIMARY TASKS AND DUTIES

  • Assists management in identifying underlying barriers to access for new patients.
  • Helps to design successful points of service delivery & outreach; i.e. building relationships with new referral sources.
  • Works with program management to create new avenues and methods of outreach.
  • Accurately evaluates the needs of referred clients from different sources, facing different obstacles.
  • Functions as patient advocate while exercising good judgment and limit setting and maintaining professionalism with each client.
  • Suggests new ways to meet the needs of patients; pursues these options with creativity.
  • Monitors timeliness of response to various types of clients; communicates effectively with team members and other staff involved in program to remove barriers and insure timely response.
  • Brings clients, parents, etc. into active contact with clinical staff, etc.
  • Verifies that clients completed care attended appointments, followup, etc.; conducts necessary followup with clients.
  • Understands and markets various services and programs to clients, based on casebycase client interviews and individual circumstances
    .

  • Functions effectively as a member of an interprofessional team that includes behavioral health, primary care providers, dieticians, pharmacists, consumers, and family members.
  • Establishes rapport quickly and communicates effectively with consumers, family members, and other providers using active listening in a nonjudgmental manner.
  • Use focused interventions to engage consumers and increase their desire to improve their health
  • Provides emergency assessment of patients in crisis and facilitates psychiatric hospitalization if needed.
  • Create and periodically update integrated care plans in consultation with health consumers, family members, and providers.
  • Provides a range of brief focused prevention, intervention and recovery services using motivational interventions, health promotion and wellness services, health education, and crisis intervention.
  • Provides referrals for internal and external services for those consumers with persistent illnesses that require followup over time.
  • Provides consultation and treatment recommendations to primary care providers in the areas of behavioral health and substance abuse issues.

ESSENTIAL FUNCTIONS/KEY COMPETENCIES

  • Knowledge of professional counseling principles, methodology, and ethics and of human psychosocial development within the family, community, and culture.
  • Demonstrated ability to work in Athena and use other office equipment effectively.
  • Familiarity with brief therapy theories/techniques and therapeutic process. Understanding of techniques for facilitating client motivation to change behavior.
  • Knowledge of health and social issues impacting diverse clients and their wellbeing.
  • Possess skills in crisis intervention, client advocacy, case management and evidence base treatment
  • Ability to interact effectively as a member of interdisciplinary healthcare team, identify and utilize community resources and communicate appropriately with diverse patient population.
