Are you passionate about helping others and making a difference in people's lives? We have an exciting opportunity for a
Case Manager to join as a member of the treatment team, functioning as the primary coordinator with clients to establish all aspects of discharge planning.


Responsibilities:


  • Under the immediate supervision of the Clinical Manager, the Case Manager is responsible for each component of the client's treatment planning, client's behavioral health plan, including all programmatic implementation.
  • Coordinate aftercare plans such as MAT service appointments, mental health continuums, primary physician appointments, relapse prevention support within the community including NA/AA meetings and identify transportation for client at time of discharge.
  • Meet with their assigned case load individually every 7 days to adhere to documentation requirements through Medicaid standards.
  • Documentation is essential as responsibilities include weekly Progress Notes, developing Relapse Prevention Plans and identifying Individual Aftercare Planning. Competency in usage of DSM-V and documentation and reporting in EMR.
  • Adhere to HIPAA compliance in all communication regarding clients.
  • Additional duties as directed.

Requirements:


  • Bachelor's Degree in a Behavioral Health discipline or related field preferred.
  • The ability to prioritize and multitask, with exceptional time management skills.
  • Ability to competently encourage and inspire direct reports in the delivery of exceptional clinical services through professionalism.
  • Demonstration of a clientcentered team approach, with advanced computer skills.
  • Must hold a valid driver's license with acceptable driving record and provide own insured transportation.
  • Excellent problemsolving and criticalthinking abilities to effectively handle challenging situations.
  • Demonstrated ability to work effectively as a part of a multidisciplinary team.
  • Understanding of Substance Use Disorders and Dual Diagnosis
  • Flexible schedule with the ability to work evenings, weekends, and holidays as needed.

Compensation:
$17-$20/ hour
