    Case Manager I

    Conifer Park, a private healthcare system, provides quality professional treatment to those suffering from Substance Use Disorders, is seeking a Clinical Case Manager. We are seeking someone that shares our values of meeting the individual needs of our patient population, being flexible and innovative to meet the needs of all who interact with us, working in a cooperative mutually supportive way, with an emphasis on continuous improvement and collaboration between departments and disciplines.

    As Case Manager you will assume full charge of patient case management, delivering individual and group mental health and chemical dependency services including assessment, discharge planning, managed care, and relevant communication, formulation and implementation of treatment plans, and performing specified duties.

    Requirements:

    High School Diploma or GED with a minimum of 1 year of Case Management experience or applicable internship and a CASAC-T certification. Chemical dependency and/or mental health experience preferred and group counseling, didactic skills.
