- Must have a current RN license in the respective state
- Minimum of one (1) year experience in in-patient nursing or hospice care for external candidates; for internal applicants, a minimum of 6 months experience as an LPN in hospice/home health
- Proficiency in working with an EMR/EHR system
- Valid driver's license and automobile insurance per Company policy
- Ability to communicate effectively in English, both verbally and in writing
- Flexibility to meet the needs of Hospice House patients
- Ability to manage the emotional challenges of caring for terminally ill patients and their families
- Physically capable of accessing patient rooms, various facilities, and homes within the service area
- BLS certification from the American Heart Association or Red Cross
- Provides support and assistance to patients and families over the phone
- Educates patients and families on available services and admission requirements
- Collaborates with the Interdisciplinary Group (IDG) for effective patient care
- Maintains proper documentation and communication
- Completes clinical admissions and initiates necessary interventions
- Develops relationships with healthcare professionals and facilities
- Successful completion of competency requirements for the position
- Provides direct patient care as part of the Interdisciplinary Group (IDG)
- Coordinates individualized Plans of Care
- Educates patients and families on various aspects of care and safety practices
- Reports patient condition changes to appropriate team members
- Attends patient deaths and provides bereavement support
- Supervises care provided by other team members
- Performs other assigned duties

The RN Care Coordinator will assess patient and family needs, coordinate the Plan of Care with the Interdisciplinary Group (IDG), and provide palliative and supportive care to the patient and their family.
