    Memorial Hermann Health System is looking for a Registered Nurse (RN) with Case Management expertise for a nursing role in Cypress, Texas.

    Job Summary

    The Case Manager role involves supporting physicians, primary medical homes, and interdisciplinary teams. By enhancing clinical outcomes, managing costs, and providing timely information to payors, you will play a key role in improving patient care quality and satisfaction. This position requires coordinating care, discharge planning, and resource utilization management, with a focus on driving positive change through performance improvement initiatives.

    Minimum Qualifications

    Education: Graduate of a nursing program required; Bachelor's of Nursing preferred, or graduate of a Master of Social Work program.

    Licenses/Certifications:
    • Active RN license in Texas
    • Master Social Worker license (LMSW) in Texas required, LCSW preferred
    • Case Management certification required within two (2) years
    Experience / Knowledge / Skills:
    • 3 years of nursing or social work experience in a hospital setting preferred
    • Experience in utilization management, case management or discharge planning preferred
    • Strong communication and negotiation skills
    • Demonstrated leadership abilities
    • Excellent analytical and PC skills
    • Current knowledge of care management, performance improvement, and managed care
    Principal Accountabilities
    • Coordinate patient care progression
    • Resolve system problems and delays in care
    • Facilitate timely diagnostic testing and treatment planning
    • Collaborate with multidisciplinary teams for patient-focused care
    • Manage discharge planning effectively
    • Participate in clinical performance improvement initiatives
    • Act as a mentor to new staff members
    • Other duties as assigned
