Position Summary:


In alignment with the Company's mission and values, the Care Provider is responsible for providing personal assistance and routine daily care and services related to the residents in accordance with the Resident Care Plan, and/or as directed by the Executive Director, Health Services Director, and Memory Care Director, in order to enhance the residents' independence and quality of life.

All responsibilities must be completed in accordance with Company policies and procedures, current standards, guidelines, and regulations.


Position Responsibilities:


  • Assist residents in their Activities of Daily Living (ADL) tasks as needed and directed. This includes transferring, bathing, grooming, personal and dental hygiene, escorting to meals and activities, bowel and bladder functions, and dressing.
  • Work as a team to respond to resident needs and alerts promptly while maintaining each resident's self
- respect, personal dignity, safety, and confidentiality.

  • Always carry radio, communicate professionally, and ensure radio is in good working order for duration of shift.
  • Assist residents with transfers to and from bed, chair, lying, sitting, standing, into and out of bed and wheelchair as needed, following safe lifting/transferring techniques.
  • Assist residents at mealtime, serving meals, clearing tables, and delivering room trays.
  • Monitor resident rooms for cleanliness and safety and report any concerns. Perform light housekeeping and laundry as needed.
  • Communicate any observed or suspected resident change of condition to a supervisor immediately.
  • Assist Activities Department in carrying out planned activities. Assist residents in preparing for activities and social programs.
  • Complete Care Provider Task Sheet for each resident by the end of shift and participate in daily crossover meetings. Review daily and update the Shift Report Log as needed.
  • Assist new residents with arranging personal items and help to familiarize them to new surroundings.

General Duties:


  • Respond to resident concerns in a professional and caring manner. Elevate issues to a supervisor when appropriate.
  • Demonstrate knowledge of each resident as an individual.
  • Maintain a safe and secure environment for all team members, residents, and guests, following established safety procedures:
  • Report occupational exposure to blood, body fluids, infectious materials, and hazardous chemicals in accordance with the Company's policies and procedures.
  • As required by state licensure, complete annual and monthly training, attend scheduled inservices and mandatory meetings. Maintain all required certifications/licensures.
  • Demonstrate flexibility in work schedule as evidenced by working holidays, evenings, weekends, and additional shifts when necessary to ensure adequate coverage within the department.
  • Display tact and friendliness when interacting with team members, residents, families, and visitors.
  • Encourage teamwork through cooperative interactions with team members and other departments.
  • Provide support to Community's marketing efforts through positive, friendly interaction during all Community tours and telephone contact with the public.
  • Ensure positive, friendly interactions with all potential team members, residents, family members, outside vendors and referral sources.
  • Complete necessary paperwork for and report any observed or suspected resident change of condition.
  • Perform other duties as assigned.

Qualifications:


  • Must be at least eighteen (18) years of age.
  • High school diploma or equivalent preferred.
  • Knowledge of and/or ability to learn the theory and practice of assisted living and dementia care.
  • Able to work with seniors and patiently interact with cognitively impaired individuals.
  • Able to operate a personal computer and be proficient in MS Office programs (Word, Excel, Outlook, PowerPoint).
  • Able to operate office equipment including, copier, telephone, postage machine, time clock, medical alert system.
  • Able to adapt and respond to change, make decisions, and prioritize tasks and projects.
  • Able to read, write and speak the English language to interact and communicate effectively with coworkers, supervisors, physicians, health care professionals, residents, and their families.
  • Must be highly motivated and able to work independently.
  • Able to obtain and maintain valid First Aid, CPR, and Food Handler's certification, as needed.
  • Must pass a Criminal Background check and Health Screening tests.
  • May require team members to vaccinate, participate in daily screening, surveillance testing, and to wear face coverings and other personal protective equipment (PPE) to prevent the spread of the COVID19 or other communicable diseases, per regulatory guidelines
