Are you looking for a full time position that offers
flexibility and a hybrid work environment (home, community, office setting).


Responsibilities:


  • Develop and implement individualized care plans for clients.
  • Educate clients on health and wellness topics.
  • Write the Plan of Care for each member.
  • Identify and link member with community resources.
  • Facilitate referrals and respond to social service needs.
  • Collaborate with other team members including doctors, nurses, other care coordinators, and clinicians.
  • Help members implement the Plan of Care.
  • Monitor and assist them in problem solving potential issues related to the health care system, financial or social barriers.
  • This roll will also support and encourage members in actively participating in reaching the goals they set together

MHRT/C Required or a year of experience in Daily Living Skills (MHRT-1 or Peer Support) ABHS keeps up on your required continued education hours for you


Pay:
$ $25.00 per hour
