We Are Hiring:Nurse Navigator - Neuroscience


The Department operates two service locations, one on the Waukesha Memorial Hospital Campus and one at the Outpatient Physician's Center in Oconomowoc.

These two locations combine to complete 400 to 500 outpatient visits monthly. Specialty services include medical pain management, pain psychology, stroke follow up clinic, Multiple Sclerosis clinic, and concussion/brain injury clinic.

The department has 2 physiatrists, 2 psychologists, 2 advanced practice nurse prescribers, 5 registered nurses and 4 clerical staff. In addition, neurosurgeons, neurologists and other medical specialists see patients within the clinics.


Schedule Details:
This position is an exempt position. Monday - Friday. No holidays or weekends.

What You Will Do:


The RN Nurse Navigator is responsible for coordinating patient and family members through diagnosis, treatment, survivorship, and end-of-life (EOL) care, ensuring smooth transitions between care settings.

The nurse navigator collaborates with a multidisciplinary team to support patients and families in addressing the following dimensions of care:
education, psychosocial needs, advocacy, continuity of care, and barriers to care.


You Will Need:


  • Bachelor's Degree in Nursing required. Master's Degree preferred.
  • Current valid State of WI RN license required.
  • At least 4 years of nursing experience required.
  • Care navigation experience preferred.
