JOB SUMMARY:


To promote the mission of PATH to help individuals achieve a more independent and fulfilling life by being an effective team member.

The ID Day Specialist will be responsible for the training and support of individuals in the program.


QUALIFICATIONS:


  • HS Diploma or GED.
  • Valid driver's license and vehicle required.
  • At least one year of experience with individuals with disabilities.
  • COVID19 vaccination, or willingness to receive vaccine required.

PHYSICAL DEMANDS:

Lifting and transferring individuals, as needed.


SPECIFIC DUTIES:


  • Assist in the coordination of training and activity planning for individuals in the program
  • Review all ISP's and be knowledgeable about individuals to provide services specific to their needs including required documentation such as Service Notes
  • Assist with the preparation for annual Adult Day Training and Office of Long-Term Living Licensing compliance
  • Meet productivity standards of the ADP Services
  • Assist in the development of assessments for assigned individuals
  • Attend, as needed, individual team meetings, ISP meetings, QEST reviews, SIS meetings, Vision for Equality monitorings and Supports Coordination monitorings
  • Meet regularly for supervision with your supervisor
