Position Overview:
We are currently seeking a motivated and detail-oriented Admissions Coordinator to join our team in the Chicagoland area.

This position plays a vital role in facilitating the admissions process for new patients, ensuring smooth transitions from referral to admission, and providing exceptional customer service to patients, families, and referral sources.


Key Responsibilities:
Serve as the primary point of contact for all incoming referrals for home health care services.

Coordinate the admissions process, including verifying insurance coverage, obtaining necessary documentation, and scheduling initial assessments.


Communicate effectively with patients, families, physicians, discharge planners, and other healthcare professionals to gather relevant information and coordinate care transitions.

Conduct thorough assessments of patients' needs and eligibility for home health care services in collaboration with clinical staff.


Provide accurate and timely updates to referral sources and ensure all necessary documentation is completed and submitted in accordance with regulatory requirements.

Participate in marketing and outreach activities to promote the agency's services and establish relationships with potential referral sources.


Qualifications:
Bachelor's degree in healthcare administration, business administration, or a related field preferred.

Previous experience in healthcare admissions, intake coordination, or customer service role preferred.

Knowledge of home health care regulations and reimbursement processes preferred.

Strong communication, interpersonal, and organizational skills.

Ability to multitask, prioritize workload, and work effectively in a fast-paced environment.
