- Collaborate with physicians, nurses, social workers, pharmacists, chaplains
- Minimum of one year of post-graduate clinical experience in palliative care, hospice, Geriatrics, and/or oncology care is strongly preferred

Description
Providence Connections Palliative Care seeks a board-certified Nurse Practitioner or Physician Assistant to join their inpatient team.This well-established, trans-disciplinary team provides a coordinated program to patients and their families for the management of chronic conditions and serious, life-limiting illness.
The ideal candidate will have previous palliative care experience. Primary location is Providence Portland Medical Center.Connections Palliative Care is part of a growing integrated Home and Community Services program which collaborates with Hospice, Home Health, Geriatrics as well as hospital-based practitioners to care for patients and their families.
