- Perform comprehensive assessments in the patients' homes and, identifies patient's needs
- Develop a plan of care in collaboration with patient/CGs, Health Care Provider, case manager, and /or Supervisor as assigned and document appropriately
- Assess the need for additional services and equipment on admission on an ongoing basis and inform the case manager and/or supervisor on a timely basis.
- Assess the home environment and make recommendations that will enhance patient care, promote safety, and prevent adverse effects
- Take verbal orders from the Health Care Provider as necessary promptly and documents appropriately
- Initiate appropriate preventive, rehabilitative, therapeutic, comfort, and pain interventions and provide instructions to patients/CGs and staff promptly
- Demonstrate professionalism by maintaining high standards of clinical care and staying current on clinical and medication issues
- Updates to patients' clinical records including medication profiles, plan of care, OASIS, case conference, progress notes, and re-certification documentation as required and on time
- Serve as a preceptor when needed
- Other duties as assigned
- Graduation from an accredited nursing program, current state license as a Registered Nurse, and CPR certification.
- Has at least one year of clinical practical nursing experience. Home Health experience preferred.
- Must have a vehicle and current driver's license and automobile insurance
- Interpersonal and communication skills necessary
- Ability to make home visits and provide skilled nursing assessments and care in the home
