Your Role


The Home Care RN Coordinator/Field Supervisor is responsible for developing plans of care, supervising caregivers in accordance with the plan of care, coordinating services for clients, and providing training, nursing care, and quality management for clients and caregivers.


Required Experience/Qualifications

  • Graduate of an accredited college or university.
  • Licensed in good standing in the State of Maryland as a Registered Nurse.
  • At least one complete year of relevant work experience in home care or in a hospital setting.
  • Understanding of Medicare and Medicaid guidelines.
  • Competent in the principles and techniques of professional nursing and required documentation that pertains to it.
  • Adept with organization, time management, and management processes.
  • Excellent constructive communication skills.

Duties

  • Assesses new clients who require or request home services and assistance with activities of daily living.
  • Schedules a new client home evaluation within 3 days of request from Employer.
  • Participates in developing the client's plan of care and in assigning appropriate personnel.
  • Participates in training and retraining the individuals who will provide the care, when indicated.
  • Coordinates total patient care by monitoring the client's conditions, promoting sound preventive practices, and coordinating services.
  • Evaluates the effectiveness of certified and personal caregivers to the patient and family on an ongoing basis.
  • Performs admission, transfer, recertification, resumption of care and discharge for the home care client.
  • Consults with client's attending physician concerning alterations of Patient Care Plans, checks with appropriate supervisor and makes changes, as appropriate.
  • Conducts initial and ongoing comprehensive assessment of the client's medical care needs.
  • Coordinates patient services.
  • Submits clinical notes no less often than weekly and progress notes and other clinical record forms outlining the services rendered as indicated.
  • Participates in case conferences, discussing with the supervisor problems concerning the patients and how they may best be handled.
  • Discuss with the appropriate supervisor the need for the involvement of other members of the health team.
  • Cooperates with other agencies to provide continuity of care.
  • Continually strives to improve his/her nursing skills by attending inservice education, through formal education, workshops, and conferences.
  • Collaborates with supervisor on company policy development, including policies for clients, patient care, and employees.
  • Adheres to all Federal, state and accreditation requirements including Medicare and Medicaid regulations.
  • Performs other job duties as reasonably required.

$ $50.00 per hour
