The Ridge Post Acute with 54 beds located in the east of San Jose, California. We understand the individuality of each person, and our healthcare staff is dedicated to achieving the maximum level of a resident's potential while providing a compassionate environment.

We are looking for a committed, caring, and reliable
Physical Therapy Assistant, PTA to join our in-house rehab team for our Skilled Nursing Facility

  • Facility Type: Skilled Nursing Facility
  • Location: 1355 Clayton Rd, San Jose, CA 9512
  • Position Type: Full Time, Part Time, Per Diem
  • Department: Rehab / Therapy
  • Pay Range: $48 $52 per hour (depends on the shift structure)

RESPONSIBILITIES:


  • Assist licensed physical therapists in evaluating and assessing patients' conditions and developing individualized treatment plans
  • Implement treatment plans under the direction of physical therapists, including conducting exercises and therapeutic activities
  • Monitor patients' progress and make necessary adjustments to treatment plans
  • Document patients' progress and maintain accurate and updated medical records
  • Communicate effectively with patients, their families, and other healthcare professionals to ensure coordinated and holistic care
  • Adhere to ethical and professional standards in all aspects of patient care
  • Stay uptodate with the latest advancements and best practices in physical therapy

REQUIREMENTS:


  • BLS and Physical Therapist Assistant Certificate
  • Interest in working with the geriatric population
  • Is in good standing with all regulatory agencies and licensing boards is required
  • Associate's degree or certification from a physical therapy program that included course study in anatomy and physiology, and clinical experience.
  • Strong understanding of human anatomy and physiology
  • Excellent communication and interpersonal skills
  • Ability to work effectively as part of a multidisciplinary team
  • Attention to detail and strong organizational skills

EXPERIENCE:


  • New passer/graduates are welcome
  • With Skilled Nursing or Long-Term Care experience is preferred
  • Experienced Physical Therapist Assistant (PTA)
