Qualifications for RN Case Managers:
- Licensed as a Registered Nurse in the state of Texas
- Reliable transportation, valid driver's license, and auto insurance
- CPR certified
- Patience, motivation, and a positive attitude
- Hospice or home health experience preferred
- Strong commitment to improving quality of life. Desire to create strong partnerships with community organizations in patient care.
- Work closely with patients on a 1:1 level while getting support from a patient care team.
Schedule:
- Monday to Friday
- Rotating On Call weekdays & Weekends
