Mobile Crisis Clinican (5k Sign-on Bonus)

The purpose of this position is to stabilize consumers in psychiatric crisis. Consumers include children, adolescents, adults, and their families.

The crisis worker will provide crisis intervention services, including information gathering, assessment, brief skill-based interventions, and safety planning, with the goal of immediate symptom reduction, stabilization, and restoration to a person-centered baseline level of functioning.

The crisis worker will assist consumers in identifying and engaging in recovery
- and resiliency-oriented goal development and achievement in a culturally aware and appropriate manner. In accordance with consumers' goals and preferences, service(s) may include one or all of the following: skill-building, identification and/or use of natural supports, use of community resources, referrals to internal or external providers of care, and facilitation of emergency services.

Duties and Responsibilities:


  • Answer crisis calls in response to mental health crises in various Chicago communities and drive to the location of the crisis within 90 minutes of receiving the call
  • Provide emergency assessments at hospitals and in the community, including consumers' homes, schools, and community centers.
  • Provide crisis intervention services to individuals in crisis, with the goal of immediate symptom reduction and restoration to a personcentered baseline level of functioning
  • Assess the client for indicators of abuse, neglect, and traumarelated diagnoses
  • Establish a provisional DSM 5 diagnosis
  • Facilitate psychiatric hospitalization when indicated
  • As appropriate, provide brief and immediate mental health services or referral, linkage, and consultation with other mental health service providers
  • Developmental crisis and mental health assessment or admission note resulting in the identification of the consumer's mental health services or referral, linkage, and consultation with other mental health services
  • Provide case management, outreach and aftercare services
  • Develop close working relationships with DCFS and DMH funded agencies, and other child and adolescent mental health service providers throughout the north and west sides of Chicago and northern suburbs
  • Work cooperatively with other members of the crisis team, including coworkers and supervisors
  • Maintain compliance with confidentiality policies and procedures
  • Maintain positive and mutually respectful relationships with other human service providers, entitlement agencies, health care vendors, and the community
  • Assist with outcome measures
  • Maintain compliance with clinical documentation, timelines and agency policies and procedures including meeting required timeframes for documentation and service notes
  • Report to assigned supervisor and actively seek consultation whenever necessary or requested by supervisor
  • Attend clinical meetings and trainings
  • Other duties as assigned by supervisor

Location and Work Conditions:


Although the crisis worker will be based out of C4's North Ave office (2542 W North Ave, Chicago, IL 60647), at least 80% of the services provided will be in Chicago's north and west neighborhoods as well as northern suburban communities.

As appropriate, for the duration of the pandemic, services may be offered remotely with utilization of HIPAA compliant telehealth related software.

Work may be stressful due to consumers' presentations, acuity of symptoms, interaction with state and local community systems, periods of high call volume, and/or tight deadlines.

The crisis worker must be willing to work certain weekends, some night or overnight shifts, and occasional holidays.

The preferred schedule(s) for this position would be Tuesday-Friday 9:30 a.m.

8:00 p.m. or Monday-Thursday 12:30 p.m. - 11:00 p.m.


Job Qualifications

  • MA/MS in Social Work, Psychology, Counseling or a related field is required.
  • Experience in counseling children and adolescents is required.
  • Personal vehicle, valid Illinois driver's license, insurable driving record, and vehicle insurance is required.
