Position Summary:


The Wellness Coordinator supports the community, and the Wellness Director/Wellness Nurse by providing clerical, administrative, planning and organization responsibilities as they pertain primarily to the nursing staff and to residents.

Only certified caregivers should apply.

Responsibilities:


  • Adheres to and assures compliance with Code of Conduct, facility policies and procedures and all applicable rules, regulations and standards as promulgated by Federal, State, and accrediting agencies or regulating bodies. This includes, but is not limited to, Department of Health, Centers of Medicare and Medicaid Services, and other applicable regulatory agencies.
  • Demonstrates the ability to assess the full scope of resident needs on a continuous basis.
  • Provides a safe environment for residents, visitors, and employees through identifying, correcting, and reporting health and safety hazards.
  • Demonstrates skill in interpersonal relationships with residents, visitors and staff.
  • Demonstrates effective oral and written communications methods
  • Assists the Wellness Director in the planning, organizing, directing, implementing, and evaluating of care and activities of the assigned care unit.
  • Communicates with medical professionals, schedules doctor appointments, and organizes transportation.
  • Maintains records and ensures compliance with regulations regarding TB testing and other employee requirements
  • Assists with staffing of caregivers
  • Assists with maintaining resident medical records including, but not limited to:
  • Filing documents in a timely manner
  • Faxing new orders to pharmacy
  • Accurate data entry of new orders into Point-ClickCare (PCC)
  • Ensuring monthly vital signs are documented and entered into PCC
  • Assist Wellness Director with Resident Care Plans for signatures and filing in a timely manner into the resident file.
  • Assist the Wellness Nurse/Wellness Director with Quarterly Medication Reconciliations for each resident.
  • Assist with New Resident Move-Ins and Move-Outs.
  • All other duties as assigned
Working Conditions

  • Works in office areas and throughout facility.
  • Moves intermittently during working hours.
  • Works beyond normal working hours on weekends and holidays, when necessary.
  • Subject to callback during emergency conditions (e.g., severe weather, evacuation, postdisaster, etc.).
  • Subject to injury from falls, burns from equipment, odors and is exposed to dust, disinfectants, hazardous chemicals, tobacco smoke, and other air contaminants.
  • Subject to hostile and emotionally upset residents, family members, personnel, visitors, or others.
  • Involved in community/civic health matters/projects.
  • Subject to exposure to infectious waste, diseases, and conditions, including Tuberculosis and the AIDS and Hepatitis B viruses.

Knowledge, Skills & Abilities:

  • Have AZ Assisted living caregiver certificate.
  • Able to perform assistance with ADLs with the residents.
  • Able to perform as a Med Tech as needed.
  • Able to operate and function with computer work.
Specific Requirements

  • Able to read, write, speak, and understand the English language.
  • Possess the ability to make independent decisions when circumstances warrant such action.
  • Possess the ability to deal tactfully with personnel, residents, visitors, and the general public based on whatever maturity level they are currently functioning.
  • Ability to be calm and levelheaded in emergencies.
  • Well groomed, professional and possess ability to work harmoniously with other personnel.
  • Ability to seek out new methods and principles and be willing to incorporate them into existing admission practices.
  • Follows written and oral instructions.
  • Maintain patience, tact, cheerful disposition and enthusiasm, as well as ability to handle residents, staff, and visitors, based on whatever maturity level they are currently functioning.
Physical Mental and Sensory Requirements (With or Without the Aid of Mechanical Devices)

  • Move intermittently throughout the work day.
  • Speak and write the English language in an understandable manner.
  • Cope with the mental and emotional stress of the position.
