CASE MANAGER


We are excited to invite a passionate and experienced
Case Manager to join our team Homes for the Homeless (HFH) is searching for an experienced Case Managers for our City Sanctuary Facility for Families w/ Children located in Staten Island, NY.

As the Case Manager, you will play a pivotal role in our organization, assisting families in providing them with the necessary care and resources needed.

ABOUT HOMES FOR THE HOMELESS, INC.

ESSENTIAL DUTIES AND RESPONSIBILITIES

  • Maintain and update digital and physical records of all families as required by governing agencies.
  • Collaborate with clients to develop service plans based on an assessment of each family's needs.
  • Assist families with connecting to community resources, support groups, and cultural services to provide social support.
  • Assisting families with referrals to outside agencies for legal services, education, parenting, medical, and mental health services.
  • Maintain and update case files biweekly to document progress.
  • Adhere to all city and state guidelines for maintaining documents.
  • Conducts unit inspections.

QUALIFICATIONS

  • Bachelor's or Associate degree.
  • Fluency in languages other than English, particularly Spanish strongly preferred.
  • Knowledge of Outlook, Word, and Excel

SALARY:

$45,000 - $50,000
