- Maintain detailed knowledge of the health care system, key service providers and health care delivery system developments as related to the health needs of residents.
- Collaborate with Resident Service Coordinators (RSC) to identify residents most in need of care coordination and self-management assistance and determine what staff member is most appropriate to provide this assistance.
- Accurately identify/assess resident assets and needs in order to connect them effectively to resources and help them set personal goals; while recognizing their knowledge and experience.
- High School Diploma or equivalent.
- Two (2) years of experience in a position that involved coordination among frequent interaction with patience/clients, health care providers, care coordinators, health plans, and other service systems critical to health.
- Bachelor's Degree in Healthcare Management, Public Health, or related field.
- Demonstrated strong understanding of how to work effectively with people with chronic disease, mental illness, disability, substance abuse and other health problems.
- Demonstrated and detailed knowledge of the health care system, key service providers and health care delivery system developments as related to the health needs of residents.
- Communicate effectively both verbally and written to a diverse group of residents, staff, and external contacts while maintaining compliance with privacy laws.

The Health Coordination Specialist will develop strategies to link residents with health care and care coordination services.Develop relationships with the primary care clinics, medical providers, care coordinators, health plans and other health service providers to increase the health and wellness of residents and reduce unnecessary healthcare utilization.
This is a grant-based position.We encourage candidates with lived experience to apply. This position is a part-time, onsite position, approximately 20 hours per week.
