We are seeking an experienced home health and hospice QA specialist to join our team.


Qualifications
Strong understanding of regulatory requirements and accreditation standards relevant to home health care services (e.g., Medicare, Medicaid, ACHC)

Excellent analytical skills with the ability to identify trends, patterns, and areas for improvement

Exceptional communication and interpersonal skills, with the ability to effectively collaborate with multidisciplinary teams and communicate complex information clearly and concisely

Detail-oriented mindset with a commitment to accuracy and thoroughness in all aspects of work

Monterey Park, CA 91754:
Relocate before starting work (required)

  • LVN with current licensure to practice professional nursing in California (preferred)
  • Proficiency in interpersonal, organizational, multitasking, time management, and communication skills
  • Excellent understanding of performance improvement and quality assurance
  • Able to take and follow through with delegated tasks and accountability
  • Strong organizational and planning skillsEthical Conduct-FlexibilityAbility to take initiative when necessary
  • Proficiency in MS Office and Google Workspace
  • Maturity and ability to deal effectively with the demands of the job
  • Team Player
  • The employee is required to have the ability to sit for extended periods of time, use hands to finger, handle or feel; and reach with hands and arms
  • This will require the ability to lift files, open filing cabinets and bend or stand on a stool as necessary

Responsibilities

  • The LVN Quality Assurance receives patient visit documentation submitted by field staff and reviews the documents for timeliness, completeness, and accuracy
  • Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions
  • Participates in administrative staff meetings and attends other meetings and seminars
  • Participates in developing departmental goals, objectives and systems
  • Maintains office services by assisting in organizing office operations and procedures
  • Performs regular chart audits and tracks appropriate trends related to risk management
  • Reviews submitted patient visit documents for completeness and accuracy in a timely manner to meet set deadlines
  • Accesses charting software to validate completeness and accuracy of submitted patient visit documents
  • Create reports and summaries in regard to status of patient visit notes submission of field staff
  • Receives the public and answers questions, in person and by telephone; responds to inquiries from employees, citizens and others and refers, when necessary, to the appropriate person, official or department
  • Operates listed office machines as required (copy machine, fax machine, time stamp machine, computer, scanner)
  • Performs other related duties as required and assignedCommunicates constructivelyListens actively

Job Types:

Pay:
$ $35.00 per hour

Expected hours: 40 per week
