JOB DESCRIPTION SUMMARY


The Certified Home Health Aide is an integral member of the interdisciplinary group who works under the supervision of a registered nurse and performs various services for a patient as necessary to meet the patient's personal needs and to promote comfort.

They are responsible for observing the patient, reporting these observations, and documenting observations and care performed.


Responsibilities:


  • Performing personal care needs after given a patient's assignment and attending to his/her requests promptly.
  • Meeting safety needs of patient and using equipment safely and properly (foot stools, side rails, Oxygen etc.)
  • Assisting in patients ADL's
  • Taking and recording temperatures, pulse, respirations, and blood pressure when ordered with appropriate skill and competency.
  • Reporting on patient's condition and significant changes to the RN Case Manager.
  • Providing respite for patient's family/caregiver when onsite, as appropriate.
  • Adhering to the organization's documentation and care procedures and standards of personal and professional conduct.

Requirements:


  • Current California Certified Home Health Aide license.
  • Ability to communicate with staff in person and via phone, maintain patient charts.
  • Intermediate proficiency in EMR system.
  • Current CPR certification.
  • CA licensed driver with an insured automobile.
  • Ability to work with little direct supervision. Secure with issues of death/dying. Provides a calm manner when in a patient's home.
  • Demonstrates tact, patience, and good personal hygiene.

Experience:


  • CHHA: 1 year (required)
  • Hospice: 1 year (preferred)
