Required Documents
- HHA Certificate (40 Hours) or Current CNA License
- Current CPR Certification (American Heart Association, Red Cross, or Health & Safety Institute)
CEU Certificates:
HIV/AIDS, Domestic Violence, Assistance with Self Meds, Alzheimer/Dementia
- Permanent Resident ID or Work Authorization ID (if applicable)
- Professional Certificate of Insurance with Liability Coverage
- Social Security Card
- Current Driver's License or ID Card
- Current Car Insurance (if applicable)
- Physical Exam within 5 months old or less. Must state you FREE OF COMMUNICABLE DISEASE.
- COVID Vaccination Card (collected if available, but not required)
- BILINGUAL ENGLISH & SPANISH PREFERRED
