Under the direct supervision of the Program Director, the Service Coordinator/QIDP provides and coordinates community-based services and supports for individuals with intellectual/developmental disabilities (I/DD) and their families, if appropriate.

The outcomes of theses services and supports are intended to improve the individuals' functioning and self-esteem, and to promote their independence, productivity, and integration in community life.

For individuals who are living with their families, desired outcomes of these services and supports include improvement in families' self-esteem and functioning; the reduction of stress; and increase in capacity for caregiving.


Responsibility:

  • Advocate, explain and protect the individual's rights and services.
  • Responsible for all service functions including assessment, planning, coordination and delivery.
  • Convene the Support Team, initiate and modify the individual services plan as part of the interdisciplinary process.
  • Assure the participation of the individual, team members, the guardian, the family and/or friends, and the necessary professional in the interdisciplinary team process, unless the individual is their own guardian and does not desire the involvement of the family or the family refuses to participate.
  • Responsible for providing or helping individuals to access the services specified in their plans.
  • Monitor and document program individual's progress in relation to the services plan.
  • Assure that services specified in the Service Plan are being implemented.
  • Identify and address gaps in service provision.
  • Serve as a liaison, when necessary, between CSS and other facilities, such as state-operated facilities, convalescent care facilities, community hospitals or rehabilitation facilities.
  • Develop natural community supports, fostering relationships with non-paid persons in the community, e.g., neighbors, volunteers, and landlords.
  • Work collaboratively with other agencies to promote the coordination of a network of services.
  • May supervise, recruit, hire, discipline, etc. Direct Support Professionals.
  • May assist individuals and families with the hiring and supervising of Direct Support Professionals, if appropriate.
  • Provide personal support and assistance in gaining access to vocational training, educational services, legal services, employment opportunities, and leisure, recreation, religious, and social activities.
  • Provide assistance in obtaining health and dental services, mental health treatment and rehabilitation services (including physical therapy and occupational therapy), and substance abuse services.
  • Access medication information including observing and reporting effects and side effects of prescribed medications; provide medication administration if/when needed and appropriate.
  • Provide support and problem-solving assistance on an ongoing basis and at times of crisis.
  • Provide informational, educational, and advocacy services to program individuals and family members.

Qualifications:
Bachelors Degree in Human Services or related field.

QIDP or Eligible to be QIDP

Valid Illinois Driver's License


Pay:
$46, $54,500.00 per year
