Job Description:
Case Manager ("Case Management Specialist") will gather initial information for Initial Assessment, Safety Planning, and Utilization Review.

Subject to approval of licensed staff, these positions provide a variety of assessment, documentation and case management services to adults.

Minimum Qualifications

Education and Experience:
Any combination of training and experience that would provide the opportunity to acquire the knowledge and abilities listed.

A typical way to obtain the required knowledge and abilities would be:

Possession of a Bachelor's Degree with significant coursework in community mental health, psychology, sociology, mental health or other closely related field of study.

Experience as a substance use counselor, or the equivalent, may be substituted for the required education on a year-for-year basis.

OR

Two years of paid experience working with adults diagnosed with a substance abuse condition and/or a serious and persistent mental illness and/or children diagnosed with serious emotional disturbances that included client assessment, documentation and case management.

Knowledge, Skills, and Abilities

Strong time management skills and excellent clinical documentation abilities required.


Working Knowledge of:
principles and techniques of case management and substance abuse interventions; client legal rights; interviewing techniques, clinical and case management documentation; counseling and basic interaction techniques related to the treatment of substance addicted or mentally/emotionally disturbed individuals; and the use of electronic health record platforms

As required by local law, Azure Acres provides a reasonable hourly range of compensation for roles that may be hired in California set forth below.
