TITLE Care Coordinator


FUNCTION


The Care Coordinator is responsible for acting as the primary conduit for the transmission of information between providers and patients.

The Care Coordinator provides appointment scheduling, patient registration, cashiering, telephone reception and triage and other related clerical functions; the Care Coordinator also assists with basic medical management data entry and referral functions.


SKILLS REQUIRED FOR THIS POSITION

  • Minimum of 1 year medical and dental office experience required.
  • Knowledge of medical terminology and surgical procedures preferred.
  • Ability to communicate effectively with patients and all levels of personnel in a professional, courteous and effective manner using excellent customer service skills.
  • Possess excellent communication and organizational skills with the ability to multi task, set priorities, and meet deadlines.
  • Results and goaloriented with a philosophy for quality improvement.
  • Must be computer literate in MS Word, Excel and Outlook; and have exceptional telephone skills.
  • Possess high job accuracy, efficiency, and dependability.
  • Must be able to maintain strict confidentiality of all personal/health sensitive information and ensure compliance of HIPAA rules and regulations.
  • High school graduate, GED or equivalent.
  • Ability to read, write, speak and understand the English language fluently; bilingual a plus.
  • Must have consistent, punctual and reliable attendance.

DUTIES AND RESPONSIBILITIES
The duties and responsibilities of the Care Coordinator may vary due to the department in which they are stationed.

It is the responsibility of every Care Coordinator to understand the function of their position as it pertains to their station.

All Care Coordinators must have the ability to perform the duties and responsibilities of the position effectively for both the Call Center and Front Desk locations.


The Care Coordinator is responsible for accurately and professionally handling all front desk operations including answering phones, greeting patients, balancing daily collections, addressing routine inquiries, and forwarding non-routine requests to appropriate personnel, and coordinate with the Call Center supervisor for scheduling appointments (emergency, private etc).

Will review scheduling concerns w/ supervisor monthly. Assist w/ red phone calls. The Care Coordinator must represent MISWM in a professional manner, following all MISWM policies and procedures.

This position is responsible for receiving incoming telephone calls in a prompt and courteous manner and performing clerical duties as directed.

The Care Coordinator will assist with the inventory and maintenance of business office supplies and the completion of daily business office reports.

Additionally, responsible for working daily insurance reports and other designated administrative reports according to procedures.

The Care Coordinator is responsible for coordination and processing of all patient referrals for specialty services. This position assists patients with external resources when needed.

Communicating with patients on a timely basis for all scheduling requirements and completing all administrative functions associated with referral activities in a timely manner.

The Care Coordinator is responsible for receiving reports and other test results, maintaining documentation, and routing to the appropriate physician promptly.
