DUTIES INCLUDE:
- Assessments
- Medication management
- Care coordination
- Acting as a liaison between healthcare professionals, participants, and families
ESSENTIAL DUTIES AND RESPONSIBILITIES:
- Conducting health histories, exams, and education
- Managing medications and diagnostic studies, coordinating care under physician supervision
- Serving as a liaison between healthcare professionals, participants, and families
- Participating in provider schedules and interdisciplinary team meetings
- Assisting in care coordination, documenting participant changes, and providing education on advanced directives
Requirements:
- Master's degree in clinical nursing from accredited Nurse practitioner program
- 35 years nursing experience with Nurse Practitioner experience preferred
- NP licensure in New Jersey and a Family certification from Nurse Practitioner program
- Meet a standardized set of competencies for the specific position description established by the state and approved by CMS prior to working
- 1 year of experience working with a frail, elderly and aging population preferred
- Must have reliable transportation, a valid driver's license, and the minimum state required liability auto insurance
- Be medically cleared for communicable diseases and have all immunizations uptodate before engaging in direct participant contact
- Pass a comprehensive criminal background check that may include, but is not limited to, federal and state Medicare/Medicaid exclusion lists, criminal history, education verification, license verification, reference check, and drug screen
