Opportunity to work for progressive patient and staff focused home care agency in Stratford, CT. Stratford VNA is seeking a RN with home care experience for a Supervisor of Clinical Services/Clinical Manager position.

Potential applicant must have a minimum of two years' experience as an RN in home health, with prior management or supervisory experience preferred.

Applicant must have a good understanding of current home health regulations and like working as a role model and team member with the ability to communicate well and motivate others.

Solid organizational skills and ability to multi-task are required.

Applicant must have a bachelor's degree in nursing or completed the required community health courses required by the Department of Public Health to be considered.


PRIMARY DUTIES AND RESPONSIBILITIES:

  • Ensure patient care to be of the highest quality, efficiency, and effectiveness by providing guidance, direction, and oversight for assigned team. Assumes 24hour accountability for administrative and clinical activities of the patient care staff.
  • Provide continuous support for patient care activities during business hours including but not limited to triage of patient visits, oversight of patient visit scheduling and support field staff.
  • Directly supervise patient care staff by managing staff productivity, perform periodic onsite visits, assist in developing appropriate care plans and discharge plans, complete annual reviews, monitor and ensure paid time off for staff, comply with supervisory guidelines for LPNs and execute disciplinary process, as needed.
  • Ensure appropriate staffing of patients through caseload and referral assignments; review referrals, updating and communicating any changes to other disciplines and the Intake Department, as needed.
  • Coordinate and execute oncall scheduling, nursing weekends in conjunction with fellow Patient Care Manager, participate in supervisor oncall rotation, as assigned.
  • Ensure staff meets quality, safety and competency requirements. Effectively execute new staff orientation, ongoing training and professional development of new and current staff. Understand and follow Stratford VNA policies.
  • Ensure staff compliance with operational policies and procedures.
  • Ensure the accuracy of OASIS and outcomes by educating staff, along with the agency OASIS Specialist, performing documentation reviews and training, and monitoring case weights.
  • Promote and demonstrate Corporate Values. Build team morale for optimal staff retention through knowledge of and strict adherence to Stratford VNA's Mission, Vision, and Core Values.
  • Participate on Performance Improvement Committee and carry out all necessary followup activities as related to quality improvement, clinical education, and utilization review, as appropriate.
  • Coordinate staff meetings in conjunction with other managers to provide education to all visiting staff regarding documentation guidelines, regulatory guidelines, clinical practice guidelines, case management guidelines, new staff orientation, and other quality improvement initiatives.
  • Maintain computer proficiency including but not limited to Stratford VNA's EMR system, SHP and other management systems.

QUALIFICATIONS:

Education/Experience:
Graduate of accredited school of nursing, with current State of Connecticut RN license.


Bachelor's degree in nursing preferred or successful completion of community health nursing courses required by the Department of Public Health for Patient Care Manager position.

Minimum two years of home health experience as an RN required. Prior management experience preferred.

Working knowledge of OASIS and home health regulations.

Must be forward thinking and possess good problem solving skills.

Ability to articulate well and present ideas well in a group.

Ability to work independently and within a multidisciplinary team.

Assures cost effective quality care.

Valid driver's license and proof of valid/current insurance required.
