Description
The Medical Transporter provides programmatic support to the medical needs of the individuals supp01ted within Vanderheyden's OPWDD certified homes.

The medical transporter is responsible for assisting to get individuals to and from needed medical care and communicating to their support team any follow up required.


Primary Duties & Essential Functions:


  • Maintains the ability to pass medications in accordance with OPWDD requirements.
  • Maintains a master appointment scheduler for OPWDD certified home.
  • Assist with providing transport and supervision to individuals for medical appointments
  • Chairs a monthly appointment scheduling meeting.
  • Reviews upcoming appointments weekly and verifies transportation arrangements
  • Ensures appointments are rescheduled when needed.
  • Reviews consults weekly and verifies required follow up appointments are made.
  • Communicates to the Individual's treatment team doctor recommendations.
  • Provides Direct Care Support as needed/assigned.
  • Completes required trainings in accordance with OPWDD standards.
  • Other duties and responsibilities as assigned by supervisor or designee.

Requirements:


  • High School Diploma/GED required

FAMILY FOCUSED- TRAUMA INFORMED- COMMUNITY BASED

  • Ability to maintain a flexible schedule
  • Must have intermediate skills in Microsoft word and excel
  • Demonstrated skills in writing

Abilities and Working Conditions:


  • Must have a valid NYS driver's license
  • Must be able to perform restraints and maintain SCIP/Promote certification.
  • Willingness to respond to the needs of a culturally diverse population.
  • Ability to work out in the community (mobile program).
  • Ability to deal positively with changing priorities as part of growth and change in a fast paced environment.
  • Display a positive, professional and proactive attitude to accomplish quality work that is in line with the agency's mission statement and sanctuary philosophy.
