Patient Types:


  • Staff will primarily be seeing patients in the community setting, i.e. their home, the home of a family member, Assisted Living Facilities, Group Homes, Nursing Homes, and perhaps a hospice patient

Patient Ratios:


  • Remote/Field: Caseload is 1315, visits per day prorated (56 visits per 8 hour shift/day)

Job Types:
Full-time, Contract


Pay:
$ $80.00 per hour

  • Hospice case manager: 3 years (required)
  • Case management: 2 years (required)
