Description

Social Worker - Full Time

Responsible for social care plans, with physicians and other interdisciplinary team members, in the provision of quality services to the residents and their families.

Education:

A Bachelor's degree in Social Work or a Bachelor's degree in a human services field, including but not limited to sociology, special education, rehabilitation counseling and psychology.

Experience: 0 - 1 year experience in social work within a nursing home environment.

Certification:
With State of Wisconsin. Additional CEU credits per State mandate.
