Part Time, Days, Benefits-Eligible - 2 weekdays, 8am-4:30pm


Responsibilities:

The Registered Nurse, OB/GYN provides prenatal nursing and educational services to women enrolled in the Healthy Beginnings Plus (HBP) Program.

The Registerd Nurse, OB/GYN demonstrates competent knowledge and skills to ensure proper assessment and care of adolescent and adult patients served.

The Registered Nurse, OB/GYN develops systems of care that monitor patient progress and promote early intervention in acute care situations.

Essential Functions:


  • Identifies the targeted population within the practice and riskstratifies all members to prioritize needs and direct interventions.
  • Assesses the healthcare, educational and psychosocial needs of the member/family. Designs an individualized plan of care with the patient and fosters a team approach by working collaboratively with the patient, family, providers and other members of the healthcare team to ensure coordination of services.
  • Coordinates appropriate laboratory and diagnostic testing.
  • Coordinates care with appropriate community health and social service resources.
  • Implements systems of care that facilitate close monitoring of highrisk patients to prevent and/or intervene early.
  • Continuously evaluates laboratory results, diagnostic tests, utilization patterns and other metrics to monitor quality and efficiency results for assigned population.
  • Incorporates excellent written, verbal and listening communication skills, positive relationshipbuilding skills and critical analysis skills into case management practice.
  • Utilizes appropriate conflict resolution, assertiveness, negotiation and collaboration skills in facilitating member throughout the healthcare continuum.
  • Maintains confidentiality and follows proper release of information as mandated by the organization.

Qualifications:

EDUCATION AND EXPERIENCE**:

  • Associate's degree in Nursing and over 6 months to 1 year as an acute care Registered Nurse required.
  • BSN preferred.

LICENSES AND CERTIFICATIONS:


  • Registered Nurse in PA licensed, upon hire, required.
