- Must be a graduate of an approved school of nursing and currently licensed in the state of the HHA
- 1- year of skilled Home Health experience required
- High tech skills, IV certification a plus
- Proven experience as LPN or similar role
- Ability to undertake a variety of diverse care tasks and work in a multidisciplinary team
- Understanding of confidentiality obligations and nursing best practices
- Excellent communication and interpersonal skills

In this Licensed Practical Nurse role, you will be providing in home high quality skilled nursing care to patients in the
Boca, Boynton, Delray area, FLarea. Under the direction of the Plan of Treatment established with a physician and with input from patient/caregiver, you will assist in ensuring a better way of living and healthy recovery. As an LPN, you would report problems/potential problems to RN/Case Manager.
*Must be willing to cover patients in the Boca, Boynton, Delray area.
